

Lisa Graham is a Registered Nurse and a certified diabetes care and education specialist with over 30 years of healthcare experience. Lisa received her Bachelor of Science in Nursing from the University of Southern California and her Master's in Integrative Health and Wellness from the University of West Georgia. Lisa served as Coordinator of the Northside Hospital Diabetes and Nutrition Education Program until her departure to start her own company. Lisa is the CEO of Renew and Live, LLC, a company that specializes in providing patient education focused on chronic disease management, particularly in diabetes and cardiovascular health. Her company contracts with physician groups, managed care organizations, state and local health departments, and community organizations to provide diabetes education in their facilities. She believes we can no longer wait for patients to come to us and that we also need to make health education accessible to patients in various formats to help improve patient outcomes.
Renew and Live’s education programs are accredited by the American Association of Diabetes Care & Education Specialists. They are a national provider of the CDC Diabetes Prevention Program, where they have achieved Full-Plus Recognition. She is also the founder/president of Diabetes You Can Win Foundation, Inc., a nonprofit that supplies clinical support for people living with diabetes in under-resourced communities.

Cheryl Smith leads Senior Direct Source, Inc. (a nonprofit based in the Greater Atlanta area) as Executive Director and CEO. With deep roots in senior advocacy, she champions aging-in-place services, ranging from healthcare and housing support to community engagement, delivered directly into senior centers, residential facilities, and other community hubs.
Under her leadership, the organization has built a robust network of partners, including health providers, insurance plans, and local civic agencies like the Georgia Department of Human Services and Atlanta Regional Commission, to deliver compassionate, accessible, and holistic aging services
Cheryl combines hands-on program delivery with strong organizational stewardship: overseeing grant-funded initiatives, community collaborations, and volunteer coordination.
